Job summary

Health Care for the Homeless in Baltimore, MD is seeking a Therapist Case Manager to provide direct mental health therapy and intensive case management for adults experiencing homelessness with complex needs.

You will partner with clients to support health, housing stability, and personal goals using a trauma-informed, client-centered approach, delivering therapy and coordinating care with interdisciplinary teams.

Qualifications

• Master’s degree in Social Work (LMSW or LCSW-C) required.
• At least 1 year of experience in therapy and case management for individuals with substance use and mental health disorders preferred.
• Knowledge of Harm Reduction, Motivational Interviewing, and Housing First practices.
• Strong communication, organizational, and crisis intervention skills.
• Valid Maryland driver’s license and access to a personal vehicle.

Responsibilities

• Deliver behavioral health assessments, individual and group therapy, and intensive case management.
• Conduct home and community visits to ensure client progress and well-being.
• Collaborate with interdisciplinary teams and external partners to coordinate care.
• Support clients with budgeting and financial literacy as part of representative payee services.
• Maintain thorough and timely documentation in the electronic health record.
