Job summary

Health Care for the Homeless in Baltimore, MD is seeking a Therapist Case Manager to provide mental health therapy and intensive case management for adults experiencing homelessness. You will collaborate with clients to support health, housing stability, and personal goal achievement through trauma-informed, client-centered care.

The role requires a Master’s in Social Work and LMSW/LCSW-C licensure, with preference for at least 1 year of relevant experience in substance use and mental health

Qualifications

  • Master’s degree in Social Work (LMSW or LCSW-C required).
  • At least 1 year of therapy and case management experience.
  • Knowledge of Harm Reduction, Motivational Interviewing, and Housing First practices.
  • Strong communication, organizational, and crisis intervention skills.
  • Valid Maryland driver’s license and access to a personal vehicle.

Responsibilities

  • Deliver behavioral health assessments, individual and group therapy, and intensive case management.
  • Conduct home and community visits to ensure client progress and well-being.
  • Collaborate with interdisciplinary teams and external partners to coordinate care.
  • Support clients with budgeting and financial literacy as part of representative payee services.
  • Maintain thorough and timely documentation in the electronic health record.

As a Therapist Case Manager, you will provide direct mental health therapy and intensive case management services to adults experiencing homelessness with complex needs.
