Job summary

Qualifications

• Minimum of two to four years of experience in social and human services or related field.
• Hands-on experience dealing with diverse populations.
• Knowledge of principles and methods of case management.

Responsibilities

• Maintain active caseload of 15 clients needing intensive case management.
• Assist in implementing service plans within 30 days of client's entry.
• Monitor client progress in substance use recovery and mental health status.
