Job summary

Health Care for the Homeless in Baltimore, MD is seeking a Therapist Case Manager to provide mental health therapy and intensive case management for adults experiencing homelessness. The role requires a Master’s in Social Work and LMSW/LCSW-C licensure, with preference for at least 1 year of relevant experience in substance use and mental health

Qualifications

• Master’s degree in Social Work (LMSW or LCSW-C required).
• At least 1 year of therapy and case management experience.
• Knowledge of Harm Reduction, Motivational Interviewing, and Housing First practices.
• Strong communication, organizational, and crisis intervention skills.
• Valid Maryland driver’s license and access to a personal vehicle.

Responsibilities

• Deliver behavioral health assessments, individual and group therapy, and intensive case management.
• Conduct home and community visits to ensure client progress and well-being.
• Collaborate with interdisciplinary teams and external partners to coordinate care.
• Support clients with budgeting and financial literacy as part of representative payee services.
• Maintain thorough and timely documentation in the electronic health record.
