Job summary

Freeman Health System's Ozark Center is seeking a Support Coach to deliver direct care services to individuals facing behavioral health challenges, with a trauma-informed, hopeful approach. You will teach and role-model skills to maximize independence and support dual recovery.

Responsibilities include supervising daily activities, preparing meals and medications, recording progress, facilitating social skills, and documenting in required records while complying with CARF and state/federal

Qualifications

• High school diploma or equivalent required.
• Six months full-time caregiving experience or one year caring for vulnerable individuals.
• Formal training in nursing arts or CNA/Home Health Aide preferred or substitution.

Responsibilities

• Ensure clients are where they should be, encourage participation in activities, and record progress notes.
• Prepare and serve meals and snacks per menu plans and timelines.
• Assist with medications per MAR and physician orders.
• Guide clients in daily tasks per the IRST program and treatment plan.
• Document paperwork accurately and report issues to supervisor.
