Job summary

Gateway Rehab Center in Erie, PA is seeking a Full-time Therapist to provide substance use treatment and case management within the Back on Track program. You will collaborate with a treatment team to deliver individual, group, and family therapy and ensure timely documentation.

The role requires trauma-informed care, completing assessments, coordinating referrals, and maintaining HIPAA compliance. A Bachelor’s degree with 1 year in health and human services is required; a PA background check

Qualifications

• Bachelor’s Degree with 1 year experience in health and human services.
• Ability to perform trauma-informed assessments and therapy.

Responsibilities

• Utilizes trauma-informed treatment with evidence-based practices.
• Develops patient treatment plans with the treatment team.
• Provides individual, group, and/or family therapy as needed.
• Performs ongoing assessments and revises treatment plans.
• Completes clinical documentation accurately and timely.
• Maintains contact with referral sources and coordinates continued care.
• Monitors insurance authorizations and referrals as required.
• Adheres to HIPAA and relevant regulations.
