Job summary

StartCare is seeking an Advanced Practice Practitioner to deliver evidence-based care for patients with substance use disorders within our CCP. You will join a medicine department team focused on care delivery, operations, and program development in New York.

The APP will identify and diagnose DSM-5 substance use and mental health disorders and tailor individualized treatment plans in line with current guidelines and SAMHSA/DEA regulations.

Qualifications

• Masters degree in nursing or physician assistant studies.
• Active NY NP or PA license; DEA registration to prescribe controlled substances.

Responsibilities

• Perform diagnostic assessments and develop trauma-informed treatment plans.
• Conduct history taking, exams, and supporting labs to guide care.
• Manage chronic conditions and co-occurring mental health disorders.
• Coordinate care within Hepatitis C and HIV programs.
• Provide urgent and emergency care as needed and participate in on-call.
