Job summary

Appalachian Mountain Health is seeking a Substance Use Care Coordinator to assist SAIOP counselors in coordinating services across care levels and supporting patients recovering from addiction. You will help organize and host daily groups and support the clinical lead during sessions.

Requirements include a Bachelor's degree and 2+ years in mental health/substance use treatment, with strong interpersonal skills and experience documenting in EHRs.

Qualifications

• Bachelor's degree or equivalent required.
• 2+ years’ experience in mental health and substance use treatment field required.
• Experience coordinating group therapy and running therapy groups preferred.
• Knowledge of community resources and public benefits helpful.
• Familiarity with federal and state health regulations and documentation practices.

Responsibilities

• Coordinate SAIOP services with LCAS and program staff.
• Co-facilitate SAIOP groups weekly with program counselor.
• Assist patients in accessing care resources and referrals to services.
• Track SAIOP hours and maintain patient-specific documentation in EHRs.
• Adhere to AMH policies, ethics, and attend required meetings.
