Job summary

The Group Health Cooperative of South Central Wisconsin is seeking a Substance Use Disorder Advanced Practice Provider (SUD APP) to join a collaborative, multidisciplinary team. This 0.5 FTE (20 hours/week) position focuses on outpatient evaluation, treatment, education and consultation for patients with substance use disorders in our behavioral health and primary care settings.

The SUD APP will prescribe and manage medications, coordinate care with physicians, nurses and counselors, and

Qualifications

• Wisconsin RN license and APNP certification with prescriber authority.
• Wisconsin PA-C license with PA-C certification.
• CPR/AED certification within four months of employment.
• 3+ years in substance use disorders evaluation and treatment preferred.
• Knowledge of evidence-based addiction medicine and pharmacology.

Responsibilities

• Outpatient evaluation, treatment, education and consultation for SUD patients.
• Prescribe and manage medications for SUD and related conditions.
• Collaborate with behavioral health and primary care teams for coordinated care.
• Document in EMR and participate in quality improvement initiatives.
