Job summary

Helping Hands is seeking a Case Manager for our reentry program to build supportive relationships with participants, help them access resources, and guide them toward long-term housing stability. You will develop Individual Reentry Plans (IRPs), monitor progress, and coordinate with partners across healthcare, housing, and social services.

The role is full-time and includes benefits such as medical, dental, vision, PTO, holidays, and a 401(k).

Qualifications

  • Strong interpersonal, verbal, and written communication skills.
  • Careful documentation and basic use of technology, including email, Word and Excel, and data entry.
  • Respectful, person-centered and trauma-informed approach; ability to work collaboratively with participants and partners.

Responsibilities

  • Build rapport with participants and learn about their needs through conversations and records review.
  • Meet with participants monthly to develop and update IRPs, set goals, review progress, and identify next steps.
  • Help participants understand and meet program requirements.
  • Assist participants in accessing housing, healthcare, SNAP, DMV appointments, employment supports, and other services.
  • Coordinate with professionals and community partners involved in participants’ success.
  • Respond to crises using trauma-informed de-escalation and connect to timely resources.
  • Develop and maintain a network of resources including medical, mental health, legal, and social supports.
  • Build relationships with landlords, housing authorities, and affordable-housing partners; stay informed about housing opportunities.
  • Provide education and resources to help secure and maintain permanent housing, including rental assistance applications.
  • Maintain accurate documentation and protect participant confidentiality.
