Job summary

Talentify is seeking a Care Coordinator to assist individuals and families in developing skills, accessing resources, and managing mental health or substance use conditions. The role emphasizes recovery, wellness, and community integration.

You will educate clients, coordinate services, and support housing, employment, and daily living goals while following agency standards and documentation requirements.

Qualifications

• Master’s or Bachelor’s degree in a human services field or equivalent.
• Qualified Addiction Professional (QAP) certification.
• Associate of Applied Science in Behavioral Health Support.
• Bachelor’s in non-human-services with two years of qualifying experience.
• Four years of qualifying experience.

Responsibilities

• Provide education to person(s)-served to learn about and manage mental illness and/ or substance use disorder.
• Provide transportation for person(s)-served when medically necessary.
• Link person(s)-served with community resources and help develop natural supports.
• Help person(s)-served develop interpersonal, social, family, community and independent living skills.
• Complete required documentation in a timely manner.
• Adhere to evidence-based practice models.
• Understand diagnostic criteria for substance use and/or mental disorders.
• Communicate information effectively and accurately.
• Ensure program aligns with agency certification standards.
• Follow program and agency policies and procedures.
