Job summary

Responsibilities

• Providing quality services and case management to assigned patients in accordance with the Recovery Navigator model.
• Performing intake assessments on all new patients to the program and completing all associated documentation within required time frames.
• Working with the patient to develop and implement an individualized plan with goals and objectives based on identified needs.
• Assisting patients with scheduling and coordinating medical and behavioral health appointments.
• Coordinating care and the plan with existing providers including but not limited to behavioral health, primary care, and community partners.
