Job summary

Groups Recover Together is seeking a dedicated case management/care coordination professional to support members on their recovery journey. You will coordinate care, connect members to appropriate services, and work closely with clinical leaders to ensure member success.

The role requires empathy, strong communication, and organizational skills, with experience in EHR systems and a valid driver's license for possible travel. This is a schedule with onsite and remote work options.

Qualifications

• High school diploma or equivalent is required.
• At least 2 years of experience in case management, care coordination, or a related role.
• Strong communication, empathy and organizational skills.
• A genuine desire to help people achieve recovery and stability.
• Experience using Electronic Health Records (EHR).
• Reliable internet access and strong WiFi for remote work.
• Must meet state-specific background screening and pre-employment requirements.
• Valid driver's license and access to a vehicle if travel is required.
