Job summary

Integrated Services Network is seeking a Full-Time Personal Agent in Saint Helens, OR to provide trauma‑informed case management for individuals with developmental disabilities. You will help identify needs, coordinate services, and support goal achievement in community settings.

The role requires teamwork, travel, and proficiency with Microsoft Office. Benefits include PTO, health coverage, HSA/FSA, and ongoing training. This is a non-exempt, on-site position with a Monday–Friday schedule.

Qualifications

• Bachelor's degree or higher in behavioral science, social science, or closely related field.
• Associate degree plus two years of human services experience.
• High school diploma with four years of human services experience.

Responsibilities

• Provide trauma-informed case management to individuals with developmental disabilities.
• Assist individuals in identifying needs and life goals and access services.
• Maintain up-to-date claims, case notes, and ISP/Annual Plans weekly.
• Conduct individual and group meetings in homes or community locations.
