Job summary

TAG MedStaffing in Brooklyn, NY seeks a Community Outreach Manager to lead street outreach, case management, and housing placement services for individuals experiencing homelessness. This leadership role supervises frontline staff and partners with community organizations and healthcare providers to deliver trauma-informed, client-centered services.

The ideal candidate is a licensed social worker with supervisory experience and a passion for leading teams that support vulnerable populations.

Qualifications

Master's degree in Social Work is required.
Active LMSW license is required.
At least 2+ years of supervisory experience in social services.
Knowledge of Housing First, Harm Reduction, trauma-informed care, crisis intervention, and case management.
Proficiency with Microsoft Office; experience with case management databases preferred.

Responsibilities

Oversee daily outreach operations and supervise Team Leaders.
Provide coaching and supervision to staff on crisis intervention and housing placement.
Coordinate services with community organizations and healthcare partners.
Lead during critical incidents and support risk assessments and documentation.
Collaborate with internal teams to improve client outcomes and ensure compliance.
Monitor program performance and support outreach and housing placement initiatives.
