Family Care Network in San Luis Obispo is seeking a Social Worker I or II to assess client needs, develop care plans, and deliver therapeutic services across homes, schools, and community settings. Qualifications

• Social Worker I: Associate degree required; BA in SW/psych preferred.
• One year of direct experience in behavioral health or social services preferred.
• Social Worker II: Associate degree and six years' experience or BA and four years in behavioral health.

Responsabilités

• Conduct comprehensive assessments to evaluate client mental health, emotional well-being, and social functioning.
• Develop individualized care plans in collaboration with clients to address needs and goals.
• Provide direct behavioral support, evidence-based interventions, and life-skill development.
• Coordinate team meetings and collaborate with families, caregivers, teachers, and other professionals.
