Job summary

Qualifications

• Experience with programs serving individuals with mental health disabilities and substance use disorder.
• Clinical training or expertise in behavioral health.
• Experience engaging participants with barriers and involvement with the criminal legal system.
• Ability to communicate and work with staff from diverse backgrounds.
• Ability to work with participants displaying a wide range of behaviors.
• Commitment to cooperation and continuity across programs.
• Ability to train staff and supervise in electronic case management systems.

Responsibilities

• Outreach and coordination with unsheltered individuals.
• Coordinate referrals to shelter or other services.
• Participate in outreach case conferences with other providers.
• Collaborate with Housing Case Manager for housing placement and related assessments.
• Engage community partners and government entities to address client needs.
