Qualifications

• High school diploma or equivalent.
• At least one year in a social services program.
• Fluent in English and Spanish desirable.
• Texas driver’s license and liability insurance required.

Responsibilities

• Ensure clients have access to all five pillars of the social determinants of health with a trauma-informed lens.
• Provide crisis intervention case management to stabilize clients.
• Conduct an initial intake and eligibility screening with potential service recipients.
• Manage client activities: schedule appointments, document, and enroll in classes or activities.
• Create and maintain service recipient records and data for monthly/quarterly/annual reports.
• Dismiss files and provide aftercare to participants.
• Maintain confidentiality per agency standards.
• Attend agency and staff meetings as assigned.
