Family Service Association of San Antonio, Inc. seeks an Intake Coordinator/Enrollment Specialist to identify and coordinate services for individuals, aligning care with Social Determinants of Health through trauma-informed practice.

You will conduct intake and eligibility screening, schedule and document appointments, maintain client records, and provide crisis intervention case management as needed. Bilingual English/Spanish skills are desirable to serve diverse communities.

Qualifications

  • High school diploma or equivalent.
  • At least one year’s paid or volunteer experience in a social service program.
  • Demonstrated knowledge of basic community resources and making referrals/follow-up.
  • Experience understanding cultural differences and sensitivity to culture in service delivery.
  • Fluent in English and Spanish (read/write/speak) desirable.

Responsibilities

  • Ensures clients have access to all five pillars of the social determinants of health with trauma-informed care through agency services and/or partners.
  • Provide crisis intervention case management services to ensure client stability and safety.
  • Conduct an initial intake and eligibility screening.
  • Manage client activities such as scheduling appointments, documenting schedules, and registering for classes/activities.
  • Create and maintain service recipient records and database for reports; input demographic information.
  • Maintain confidentiality in accordance with agency standards.
  • Attend agency/program staff meetings and peer consultations.

The Intake Coordinator/Enrollment Specialist identifies and directly coordinates, provides contacts, refers or otherwise arranges for individuals to receive services as needed, while ensuring that services provided are in alignment with the Social Determinants of Health through a trauma-informed care lens as defined by the agency.

Job Functions:
  • Ensures that clients have access, as needed, to all five pillars of the social determinants of health with a trauma-informed lens through Agency services and/or community partners.
  • Provide essential services to individuals and families in crisis. Crisis intervention case management services will be provided based upon a crisis evaluation to provide intervention and stabilization services to ensure client stability and safety.
  • Conduct an initial intake and eligibility screening with potential service recipients.
  • Manage client activities, as assigned, such as scheduling client appointments, documenting scheduled appointments, and/or registering clients for classes and activities.
  • Create and maintain service recipient records and database for monthly, quarterly, and annual statistical reports; inputting client demographics and other information into the database.
  • Dismiss files and provide aftercare to participants.
  • Maintain service recipient and family member confidentiality in accordance with agency standards.
  • Attend agency, program and staff/team meetings, peer consultations, and other meetings as assigned.

Education/Training/Experience:
  • High school diploma or equivalent.
  • At least one (1) year’s paid or volunteer experience in a social service program.
  • Demonstrated knowledge of basic community resources and of making referrals and providing followup.
  • Experience in understanding cultural differences and is sensitive to the implications of culture for service delivery.
  • Fluent in English and Spanish (read, write, and speak) desirable.
  • State of Texas vehicle operator’s license and proof of liability insurance.
