Job summary

Ruth’s House of Hope is seeking a Case Manager to support residents of Ruth’s House and Sarah’s House, ensuring safe, stable environments and connection to community resources. This role emphasizes trauma-informed, strengths-based, and person-centered approaches to individualized service planning.

The Case Manager will work with leadership and local partners to promote housing stability, recovery, and well-being, including crisis management and policy adherence.

Qualifications

• Requires a Bachelor's degree or equivalent experience in social services.
• Experience serving people experiencing homelessness or housing instability preferred.
• Ability to provide trauma-informed, client-centered services.
• Strong written and verbal communication skills.
• Ability to work independently and as part of a team.
• Respect for diverse populations and commitment to inclusive services.

Responsibilities

• Follow admissions and intake procedures for RH and SH programs.
• Complete intake and documentation in CMS.
• Submit required information to the County in a timely manner and follow funding procedures.
• Communicate with county and government agencies about resident benefits.
• Monitor deadlines and maintain accurate documentation and adherence to policies.
• Collaborate with the Program Director on policy concerns and transition planning as needed.
