Job summary

The Road Home, a long-standing nonprofit serving the Midvale community, seeks a Housing Focused Case Manager to help individuals experiencing homelessness access stable housing. You will connect guests to community resources, coordinate with the shelter team, and develop personalized housing plans in a trauma-informed, guest-driven environment.

Qualifications

• Fluency in Spanish and English is required.
• Experience working with diverse populations preferred.
• Must pass pre-employment background check and drug screening.
• Position is in-person and requires driving to transport guests.

Responsibilities

• Help individuals experiencing homelessness secure stable housing.
• Identify housing barriers and connect guests with community resources.
• Maintain accurate records and case notes.
• Transport guests to housing appointments and viewings using agency vehicles.
• Collaborate with shelter teams to develop housing plans and strategies.
