Job summary

SELF Inc. is seeking a Housing Navigator to provide trauma-informed, housing-focused services within emergency housing programs.

You will connect participants with resources, develop individualized service plans, assist with accessing IDs, and support transitions from temporary housing to permanent supportive environments. This full-time, exempt role reports to the Program Director and requires strong communication, case management, and collaboration with community partners.

Qualifications

• High School Diploma or GED; Bachelor’s degree preferred.
• Minimum three years of homeless, mental health, or addiction services.
• Driver’s license and clean driving record preferred.
• Criminal background clearance required.

Responsibilities

• Provide direct coaching and care with a harm-reduction approach.
• Manage a caseload of 20–25 participants across programs.
• Perform initial assessments within 72 business hours of enrollment.
• Identify, screen, and arrange connections to needed services.
• Maintain HMIS/Client Tracks data entry and timely housing assessments.
• Develop individualized service plans with timelines for housing moves.
• Advocate for residents and collaborate with external agencies.
