Job summary

CTSHealth is seeking a Case Manager to lead a service program and coordinate care across multiple teams in Chicago, IL. You will liaise with Operations, clinicians, and direct supervisors to ensure trauma-informed, person-centered plans and timely service delivery.

Applicants should have a Bachelor’s degree in Human Services or Social Work and at least 3 years of crisis intervention experience, with strong communication and documentation skills.

Qualifications

• Bachelor’s degree in Human Services or Social Work.
• 3 years’ experience in Crisis Intervention (CIT) or similar stabilization methods.
• Experience with trauma-informed, person-centered care.

Responsibilities

• Coordinate case management and care planning across services.
• Maintain client confidentiality and safety as a priority.
• Document progress, referrals, and service verifications in records.
• Collaborate with clinicians, supervisors, and Case Workers to implement plans.
• Respond to emergencies or crises following established protocols.
