Job summary

NCCF is seeking a Case Manager to provide housing supportive services under the Permanent Supportive Housing Program in the District of Columbia. The role emphasizes trauma-informed, evidence-based engagement with adults and children, focused on reducing homelessness and promoting self-sufficiency.

Responsibilities include biopsychosocial assessments, service plan development, Medicaid enrollment, HMIS data entry, and coordination with internal and external providers.

Qualifications

• Bachelor's degree in Social Work, Mental Health Counseling or related field.
• Minimum of two years' professional experience in counseling or case management for homeless populations.
• Knowledge of social service delivery systems and engagement strategies.
• Strong teamwork and interpersonal skills aligned with NCCF's mission.

Responsibilities

• Provide housing supportive services and develop biopsychosocial assessments.
• Enroll participants in Medicaid and monitor benefits.
• Develop and update individualized service plans and quarterly re-certifications.
• Maintain case notes in HMIS and prepare monthly progress reports.
• Coordinate with treatment providers and refer families as needed.
• Lead monthly case review meetings and participate in trainings.
