Job summary

Shelter House is seeking an Entry‑level Community Housing Case Manager to support 15–20 families in Prevention and Rapid Rehousing programs in Fairfax County, VA. The role follows a housing‑first approach with trauma‑informed care to help clients obtain and maintain housing.

You will conduct screenings, develop housing stability plans, coordinate services with landlords and community partners, and document contacts in HMIS to support client self-sufficiency.

Qualifications

• Bachelor's degree in human services or related field or commensurate experience.
• At least 1 year of human services or direct services experience.
• Ability to prioritize competing priorities and make sound judgments.
• Strong written and oral communication skills.

Responsibilities

• Conduct comprehensive assessments and screening of housing history, barriers, health, and needs.
• Develop individualized Housing Sustainability Plans and coordinate services.
• Meet regularly with households to provide case management and referrals.
• Provide housing navigation, landlord engagement, and lease-up support.
• Maintain timely documentation in HMIS or client files within 48–72 hours.
• Collaborate with Fairfax County and partners to deliver wraparound services.
• Assist clients in overcoming barriers to housing, such as arrears or credit history.
