Job summary

St. Vincent de Paul is seeking a dedicated IHC Case Manager in Phoenix to support shelter residents with trauma-informed, strengths-based case management and individualized housing plans.

You will collaborate with residents to access services, remove barriers to permanent housing, and monitor progress toward housing, income, health, and stability goals. A driving license and experience serving vulnerable populations are preferred.

Qualifications

  • Bachelor’s degree preferred; High School diploma or GED required.
  • 1+ years case management experience (paid or internship) OR 2+ years in human services role.
  • Valid driver’s license preferred.

Responsibilities

  • Carry a caseload of residents, meeting with each resident at least weekly.
  • Complete assessments and individualized service plans to identify needs, strengths, goals, and barriers.
  • Monitor and document resident progress toward stabilization, income, and housing goals.
  • Maintain timely and accurate records in data systems within 24 hours of interaction.
  • Coordinate incident intervention and conflict resolution with staff and security as needed.
  • Assist residents in identifying and navigating community resources and housing processes.
  • Attend and participate in staff meetings, case conferencing, trainings, and agency events.
  • Support volunteers and contribute to SVdP’s Culture of Invitation.
