Qualifications

• Bachelor's degree in social work or related field required or preferred.
• Experience with case management, shelter, or human services is preferred.
• Ability to advocate for residents and coordinate services.

Responsibilities

• Carry a caseload of workforce participants, meeting with each resident weekly.
• Recruit new participants and assess eligibility for programs.
• Develop individualized service plans with residents focusing on skills and goals.
• Monitor progress with case notes, service plans, and progress reports.
• Maintain timely records in data systems within 24 hours of interaction.
• Coordinate with staff to manage incidents and conflicts, using de-escalation as needed.
• Connect clients with resources such as training, education, and certifications.
• Coordinate with employers and providers to facilitate job placement and training.
• Attend staff meetings, case conferences, trainings, and agency events.
• Support volunteers and help create engagement opportunities.
• Perform other duties as assigned by program management.
