Human Options is seeking a Case Manager to provide high‑quality, field‑based case management for clients. Responsibilities include assessments, care planning, coordination of medical and behavioral health services, and comprehensive documentation to meet grant and agency requirements.

The role requires a Bachelor's degree in a relevant field, 3+ years of direct client services, 2 years in case management with at-risk families, and preferred English/Spanish bilingual skills.

Qualifications

• Bachelor's degree required in social work, psychology, counseling, sociology, behavioral science, or commensurate experience.
• Three years of experience providing direct services to the target population.
• Two years of experience in case management with at risk children and families.
• One year's knowledge of the child welfare system preferred.
• Familiarity with electronic medical records.
• Bilingual in English and Spanish preferred.

Responsibilities

• Conduct outreach, enrollment, case management, and documentation in alignment with contract goals and expectations.
• Assess the financial, social, and emotional needs of the client.
• Develop individualized care plans to improve quality of life and health outcomes.
• Engage with clients in person, in a field-based setting, and via phone.
• Coordinate medical and behavioral health access issues and document accordingly.
