Job summary

Everstand seeks a dedicated Case Manager to coordinate trauma-informed case management for children in care. You will liaise with families, sponsors, and external providers to ensure timely, individualized services and smooth transitions.

Strong communication, organization, and cultural responsiveness are essential. Responsibilities include maintaining records, conducting assessments, and coordinating comprehensive support across medical, educational, legal, and social domains.

Qualifications

• Bachelor’s degree in behavioral sciences, human services, social services, or a related field.
• Fluency in English and Spanish.
• Child welfare and/or case management experience preferred.
• Experience working with children, youth, families, or vulnerable populations preferred.

Responsibilities

• Coordinate individualized case management services and referrals.
• Serve as primary contact for children, families, sponsors, providers, and community partners.
• Coordinate medical, behavioral health, educational, legal, recreational, and other support services.
• Complete assessments, care plans, case reviews, reports, and required documentation within set timeframes.
• Coordinate interpretation and translation services to support effective communication with children and families.
• Monitor progress toward individual goals and participate in case management meetings and staffing.
