CASES seeks a Care Manager to provide person-centered care management services, develop individualized care plans, and support members with access to services. You will use motivational interviewing and coordinate with families, focusing on trauma-informed, recovery-oriented practices.

The role includes field work and strong documentation within a health home portal. The position offers a flexible schedule (mostly 9am–5pm) with 80% fieldwork, and compensation at $50,000 annually.

Formación

• High School Diploma and CASAC certification or equivalent
• Bachelor's degree with 4 years of social services experience or related credentials
• Master's degree with 2 years of social services experience
• 2 years' experience in providing direct services
• Strong knowledge of wellness, recovery, and self-help
• Effective communication and written skills are essential
• Proficiency with computer and databases

Responsabilidades

• Complete all documentation within required time frames in the web-based health home portal
• Provide core care management services (Comprehensive Care Management, Care Coordination & Health Promotion, Comprehensive Transitional Care, Enrollee & Family Support, Referral to Community & Social Supports, Use of HIT to Link Services)
• Provide care management services using motivational interviewing techniques to support consumers to address their problems
• Ensure member has access to services per Care Plan (e.g., medical, home health, behavioral health, housing, entitlements)
• Meet minimum 4 case management contacts per month (2 in person)
• Escort members to appointments and involve family in care planning
• Respond to crisis situations as needed
• Use databases and reporting tools (GSI Health Coordinator, ePACES/eMedNY, Medicaid Analytics Portal)
• Stay informed on regulations and trainings related to health home care management and Medicaid redesign
• Provide services in a culturally sensitive, trauma-informed, recovery-oriented manner
• Other duties as defined by team lead or CASES staff
