Job summary

Community Partners in Action is seeking a Family Support Specialist for the REGIONS-Site Secure Residential Treatment program in Hamden, CT. The role supports male youth and families, coordinating with clinicians and mentors to engage families in treatment and aftercare planning. Ideal candidates have a HS diploma or GED and at least two years of experience with system-involved families.

Bilingual English/Spanish is preferred and a CT driver’s license is required. Flexible schedule incl.

Qualifications

• HS diploma or GED required; preferred lived experience.
• Bilingual (English/Spanish) preferred.
• Knowledge of gender-specific, trauma-informed care and psychosocial interventions.
• Familiarity with state and federal laws relevant to youth services.

Responsibilities

• Collaborate with Reintegration Mentor and Clinician to engage youth, family and community resources.
• Support families to participate in treatment and build a supportive network.
• Serve as a role model, promoting hope and self-advocacy.
• Develop family engagement strategies tailored to each youth/family’s needs.
• Ensure families attend visits and treatment sessions; participate in team meetings.
• Participate in monthly ITP and weekly MDT meetings; share family perspectives.
• Assist with transition planning to community after-care services.
• Complete required trainings and maintain professional boundaries.
