Job summary

Southwest Healthcare Inland Valley Hospital in Wildomar, California, seeks a qualified Social Worker to join a multidisciplinary team providing assessment, planning, and care coordination for patients. The role includes intervening with families, advocating for patient needs, and coordinating discharge planning.

The ideal candidate will have an MSW from a CSWE-accredited program (or related master’s degree) and at least 2 years of experience in a mental health setting, with acute care experience

Qualifications

• MSW from CSWE-accredited program or related master’s degree that meets position qualifications.
• 2 years as a social worker in any mental health environment required.
• Acute care experience preferred.

Responsibilities

• Provide assessment, interventions, support, advocacy and case management.
• Develop treatment plans in collaboration with the clinical team.
• Floats to other facilities within Southwest Health system as needed with orientation provided.
