Job summary

Family Care Network (FCNI) is seeking a Support Specialist to work with clients and families, under supervision, to develop and implement personalized care plans across home, school, and community settings.

The role emphasizes teaching coping skills, providing emotional support, and promoting independence. Hybrid schedule in San Luis Obispo and related counties offers meaningful, mission-driven work with telework flexibility where applicable.

Qualifications

• Associate degree required; Bachelor’s degree in Social Work, Psychology, or related field preferred.
• Six years full-time equivalent experience in Behavioral Health for Support Specialist II, or four years with a Bachelor's in related fields.
• Must be computer literate with proficiency in Microsoft Office Suite and knowledge of database and reporting tools.

Responsibilities

• Provide direct behavioral support, use evidence-based interventions, and reinforce new behaviors.
• Teach life skills including daily living activities and medication management.
• Provide psychoeducation on mental health and substance use topics.
• Engage clients in meaningful activities and assist with transportation to community resources.
• Provide guidance in coping strategies for crisis situations and safety planning.
• Train and support families and caregivers in behavior management strategies.
• Monitor client progress and adjust plans as needed; participate in after-hours on-call rotation.
