Job summary

Safe Horizon in New York is seeking a Case Manager to provide trauma-informed, client-centered advocacy and case management for domestic violence survivors and their children in a residential setting.

You will assess safety, manage risks, advocate for client benefits, document case files, and support residents through weekly sessions and team meetings. Knowledge of anti-oppressive approaches is essential.

Responsibilities

• Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients.
• Provide ongoing safety assessment and risk management, crisis intervention, crisis counseling, emotional support and case management services of complex client cases
• Advocate on behalf of clients with internal and external partners to increase client’s access to benefits and resources; provide accompaniment as necessary
• Maintain accurate and timely documentation of case files, including funder reports
• Engage residents in weekly individual case management sessions focusing on engagement, de-escalation, risk management and safety assessment
• Participate in staff meetings, case conferences, supervision meetings, trainings and other meetings as required
• Assist in the daily operations of the work site to maintain adequate coverage, and a trauma informed/healing environment
