Job summary

Iris Family Support Center is seeking a Community Partners Case Manager to work in home-based and community-based settings, providing trauma-informed services to families referred by the Department of Child Services and other community sources. You will perform intake, assessments, and ongoing case management, coordinating with partner agencies to promote family safety and stability.

The role requires independent judgment, strong communication, and the ability to manage a fluctuating caseload,

Qualifications

Bachelor's degree in a human services field is preferred.
One year of full-time home management or child-care experience is preferred.
Experience with families/parents/children in a human services setting is desirable.
Life or job experience caring for children is valued.

Responsibilities

Deliver services to referred families and maintain caseload to meet productivity requirements.
Conduct outreach to enroll clients into services through various engagement methods.
Complete intake and assessment of families within required timelines using agency databases.
Visit clients in their homes and connect families to supportive services to promote safety and stability.
Document case notes thoroughly and on time; maintain case plans and progress updates.
Attend court hearings, team meetings, and trainings as required.
Travel approximately 40% within assigned counties to conduct visits and assessments.
