Qualifications

• Proficient in timely documentation and navigation of FSFN system.
• Ability to travel regionally and statewide as needed.

Responsibilities

• Provide continuity of care and case management for children ages 0-17.
• Document progress and prepare timely reports.
• Link resources in the community and collaborate with the Family Care Team.
• Maintain accurate records in electronic and paper systems.
• Authorized to access protected health information.
