Job summary

Iris Family Support Center is hiring a Community Partners Case Manager in Indiana. The role provides home-based and community-based services to families referred by the Department of Child Services and other sources, using trauma-informed, evidence-based practices.

You will assess needs, develop service plans, and coordinate with partner agencies. Work is largely solo with supervision available, managing a dynamic caseload, conducting home visits, intakes, and ongoing case management to promote

Qualifications

• Bachelor's Degree in Human Services, Social Work, Education, or related field.
• One year of full-time experience in home management, child-care, or related field.
• Experience working with families in a human service setting preferred.

Responsibilities

• Deliver home- and community-based services to referred families.
• Maintain caseload to meet productivity requirements.
• Conduct outreach to enroll clients into services (face-to-face, calls, texts, social media).
• Provide intake and assessment of all families on caseload within required timelines and document accordingly.
• Visit individuals in homes and connect families to supportive services.
• Attend court hearings, case conferences, and team meetings as required.
