Job summary

The Black Mental Health Corporation is seeking a dedicated Case Manager to provide Community Psychiatric Supportive Treatment in Ohio. You will support clients with complex mental health needs, coordinate services across multiple systems, and ensure documentation aligns with state requirements.

Responsibilities include engagement, assessment, care coordination, skill development, and ongoing monitoring, with focus on culturally responsive, person-centered care.

Qualifications

• High School Diploma/GED and three years of relevant experience in a behavioral health setting.
• Associate’s Degree and two years of relevant behavioral health experience.
• Bachelor’s Degree in a behavioral health field; or Bachelor’s in a non-behavioral field with one year of behavioral health experience.

Responsibilities

• Engage clients with ongoing outreach, follow-up, and participation in services.
• Coordinate services with behavioral health, medical, housing, employment, and social service providers.
• Link clients to community resources including benefits, housing, transportation, crisis services, and specialists.
• Provide skill-building interventions to improve daily living, interpersonal, and coping skills.
• Monitor progress toward goals and document changes in symptoms, functioning, or needs.
• Review progress with clients and treatment team; modify treatment plans as needed.
• Maintain complete, accurate clinical records per OhioMHAS standards.
• Participate in staff meetings, supervision, and trainings.
