Job summary

Zepf Center, serving Lucas County and Northwest Ohio, seeks a Case Manager to coordinate care for individuals with serious mental illness and/or addiction. You will develop treatment plans, connect clients with community resources, and advocate for their needs in a compassionate, person-centered approach.

Qualifications

• HS diploma or equivalent with 3 years behavioral health experience OR Bachelor’s degree REQUIRED
• Valid Driver’s License with acceptable driving record
• QHMS certification required throughout employment
• Social Work license preferred

Responsibilities

• Assess client strengths and needs; develop treatment plans and monitor progress.
• Maintain contact with clients to ensure stability and help them reach goals.
• Link clients to community resources and advocate for entitlements.
• Coordinate care across agencies and teams; ensure cost-efficient services.
• Document in electronic health record within required timelines.
• Provide crisis intervention and support in collaboration with the team.
