Job summary

Concord Counseling Services in Westerville, OH is seeking a dedicated Case Manager to link clients with mental health resources, coordinate services, and advocate for individualized support plans. You will collaborate with a multidisciplinary team to reduce hospitalizations and improve daily functioning.

Responsibilities include assessing client needs, maintaining documentation in EMR, arranging transportation, teaching daily living skills, and ensuring compliance with program standards.

Qualifications

• Associate degree or related field with at least 1 year direct contact care in mental health preferred.
• Working knowledge of mental illness and its impact on functioning.
• Ability to document service notes, needs assessments, and individual service plans in EMR.
• Valid driver’s license, reliable vehicle and insurance with a good driving record.
• Ability to pass a pre-employment background check and drug screen.

Responsibilities

• Provide ongoing linkage, monitoring, planning, evaluation and advocacy for clients within medical and social service communities, law enforcement and health department; safely transport to related appointments.
• Teach and support daily living activities such as shopping, nutritious meals, and budgeting resources.
• Monitor symptoms, de-escalate behaviors and respond to crises with medical and community professionals as needed.
