Job summary

New Reach, Inc. in Connecticut is seeking a Case Worker & Housing Specialist to provide housing search, placement, stabilization, and case management for clients in Rapid Re-Housing and Project Longevity programs.

You will support individuals and families experiencing homelessness, coordinate housing subsidies and services, and document their progress while collaborating with shelters, landlords, and community partners to secure lasting housing stability.

Qualifications

• Associate's degree (bachelor's preferred) with at least two years of relevant experience.
• Bilingual in Spanish preferred; ability to work with diverse populations.
• Excellent oral and written communication skills.
• Valid CT Driver’s License, proof of insurance, and reliable transportation.
• Experience in housing, housing law, and related services.

Responsibilities

• Manage a combined caseload across Rapid Rehousing and Project Longevity per program requirements.
• Complete intake, eligibility screenings, housing assessments, and ongoing needs assessments.
• Develop individualized service and housing plans and monitor progress toward housing stability.
• Provide housing search and placement assistance and coordinate relocations.
• Recruit landlords and support sustainable tenancies.
• Maintain HMIS documentation accurately and timely.
• Coordinate referrals to benefits and community resources.
• Participate in trainings and program development.
