WestCare Inc. is seeking a dedicated Case Manager to support children and families within the child welfare system in Doral, FL. The role focuses on safety, well-being, and permanency, applying trauma-informed practices and collaborating with a team of professionals.

Responsibilities include developing case plans, conducting reviews, coordinating services, and maintaining compliant documentation. A 4-year degree is required, with a master’s preferred; certification within 1 year is expected.

• Bachelor’s degree is required; Master’s preferred.
• Certification as a Case Manager is required within 1 year from hire date.

Mansioni

• Assess the needs and strengths of children and family
• Develop Case Plans in collaboration with the children and family
• Conduct monthly Case Plan reviews
• Document all services and activities timely in accord with standards
• Coordinate child and family care with the treatment team and external providers
• Prepare Judicial Reviews Social Study report (JRSSR) and Progress Reports for courts and agencies
• Provide referrals and linkage for the children and family as needed
• Ensure HIPAA compliance and maintain confidential files
• Support permanency planning and reunification efforts
• Participate in multidisciplinary team meetings and court hearings as required
