Qualifications

• Bachelor’s degree in social work, psychology, human services, sociology, or related field required.
• Two years of experience in case management, care coordination or family support services preferred.
• Bilingual abilities are preferred to serve diverse families.

Responsibilities

• Serve as intake point for program referrals; log, track, and follow each referral through assignment and closure.
• Complete initial screenings and gather case information from caregivers and partners.
• Coordinate case assignments to therapists based on family need and caseload.
• Maintain referral and caseload tracker ensuring current status.
• Provide case management including service plans, progress monitoring, and follow-up.
• Make referrals for behavioral health, medical, educational, and community services.
• Document contacts and services in CDS/CCWIS within required timeframes.
