Youth and Family Advocates in Florida seeks an Early Childhood Case Manager to support children aged 0–5 and their families. You will provide case management from intake to discharge and link families with community resources using trauma-informed approaches in a collaborative team.

This role requires a Bachelor’s degree and CWCM certification, regular regional travel, and ability to work with Child Welfare and Early Childhood Court processes to develop and monitor permanency plans.

Qualificações

• Bachelor’s degree required.
• CWCM certification required.

Responsabilidades

• Function as a contributing member of a case management team.
• Provide case management services from intake to discharge, including assessment, planning, linking, monitoring, and advocacy.
• Increase parent engagement through visits and attachment.
• Encourage parent participation in Early Childhood Court (ECC) to overcome barriers.
• Link families to evidence-based parenting and early childhood services.
• Conduct weekly home visits for high-risk cases.
• Develop service/case plans with permanency goals and timelines.
• Review and update plans monthly or as needed.
• Provide families with information to promote child safety and family stability.
• Maintain records in the data system with timely notes.
• Prepare judicial work in accordance with Florida Statute.
• Identify and monitor at-risk children and families.
• Refer to community services to ensure continuity of care.
• Participate in staff development programs including TBRI, MI, Circle of Security.
• Other duties as assigned; access to protected health information.
