Job summary

WestCare Foundation is seeking a qualified case manager to advocate for program consumers and coordinate services across internal teams and external partners in Miami, FL. The role emphasizes evidence-based assessments, service planning, and regular reviews to support wellness and recovery goals.

Ideal candidates will have a bachelor’s degree in a related field, relevant licensure, and at least two years of supervised therapy experience.

Qualifications

• Certification as a certified case manager in good standing (preferred).
• Graduation from an accredited bachelor’s program in a related field is required.
• Two years of supervised work experience in individual/family therapy is preferred.
• Bilingual abilities are preferred.

Responsibilities

• Administer evidence-based assessments as needed.
• Develop Service Plans in collaboration with the consumer and family.
• Conduct monthly Service Plan reviews and document all services.
• Provide case management and linkage for consumers and families.
• Coordinate care with treatment teams and external providers.
• Arrange transportation for consumers as necessary.
• Prepare progress reports for courts, probation officers, and caseworkers.
