Job summary

WestCare Foundation is seeking a Case Manager in Doral, FL to support children and families involved in the child welfare system. You will implement trauma-informed, family-centered practices while ensuring safety, permanency, and coordination with treatment teams and community providers.

The role requires a 4-year degree, Florida driver’s license, and the ability to work flexible hours. Certifications and state-accepted practices are emphasized to maintain compliance with CCWIS and HIPAA.

Qualifications

• Certification as a Certified Case Manager (CBHCM or CWCM) in good standing by an agency approved and recognized by the Florida Certification Board is required within 1 year from the date of hire.
• Bachelor’s degree in social work, human services, or a related field (required).
• Master’s degree in social work or a related field (preferred).
• Minimum of two (2) years of related experience providing case management and/or care coordination services preferred.

Responsibilities

• Assess the needs and strengths of children and family.
• Develop Case Plans in collaboration with the children and family.
• Document all services and activities timely in accord with certification, licensing, Medicaid, accreditation standards and agency policy and procedure.
• Conduct individual case management sessions as needed.
• Provide referrals and linkage for children and family, as needed.
• Coordinate child and family care with the treatment team and external providers.
• Establish and maintain meaningful connections for youth with kin, mentors, and foster/adoptive families.
• Ensure accurate and timely documentation of services in compliance with CCWIS requirements.
