Job summary

Youth and Family Advocates seeks a dedicated Case Manager to support permanency for children through a trauma-informed, family-centered model. This role will coordinate assessments, planning, linking to services, and ongoing monitoring from intake through discharge in Florida.

Responsibilities include increasing parent engagement, coordinating with ECC and court processes, and maintaining thorough case documentation while ensuring compliance with Chapter 39 and agency policies.

Qualifications

• Bachelor’s degree required, preferably in human services.
• CWCM certification required.
• Familiarity with trauma-informed principles and practices.

Responsibilities

• Provide case management services from intake to discharge for children and families.
• Increase parent-child visitation and engagement in Early Childhood Court.
• Coordinate access to community resources and evidence-based services.
• Maintain accurate electronic and paper case records and timely documentation.
• Perform weekly home visits for high-risk cases and ensure 12-month permanency planning.
