Job summary

Youth and Family Advocates in Florida is seeking a Family Care Manager to provide continuity of care for children ages 0-17 within a family care management model. You will develop, expand, and link community resources while documenting progress and ensuring a trauma-informed approach across all services.

You will manage assessments, treatment plans, and monthly progress reports; collaborate with the care team and families; ensure compliance with Florida Medicaid requirements; travel regionally

Qualifications

• BSW/BA or MSW/MA in human services required; Foster Care/Adoption preferred.
• Strong verbal and written communication with clear documentation.
• Must have valid Florida driver’s license and ability to travel regionally.

Responsibilities

• Provide case management: assessment, planning, monitoring, and advocacy.
• Complete Medicaid assessments and case/treatment plans for children in foster care.
• Maintain records and monthly progress reports; coordinate community resources.
