Job summary

Metro Nashville is seeking a case manager to provide trauma-informed services to victims of interpersonal violence at the Office of Family Safety. You will screen, assess, and plan individualized care, coordinating with a multidisciplinary team to connect clients with needed resources and support throughout legal and community processes.

Qualifications

• Bachelor's degree from an accredited college or university.
• Master's degree preferred for related field.

Responsibilities

• Provide comprehensive trauma-informed case management to victims served by the Family Safety Center.
• Pre-screen referred individuals to assess program applicability.
• Determine needs and formulate weekly and monthly case plans.
• Manage client intakes, session documentation, and assessments.
• Monitor clients' progress toward client-centered goals.
• Advocate and refer clients to community resources.
• Collaborate with a multidisciplinary team to monitor progress.
• Facilitate effective communication with service providers and clients.
• Identify and foster meaningful community partnerships with providers.
• Provide weekly community-based client meetings.
• Provide support for clients in legal proceedings as needed.
• Input and manage client files and data timely.
• Engage in ongoing client meetings and trainings.
