Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or related field.
• Three years of progressively responsible experience providing direct case management services.
• Trauma-informed and safety-focused practices are preferred.

Responsibilities

• Interviews, accepts, and provides comprehensive, long-term case management services for an assigned caseload.
• Develops individualized service plans and monitors client progress.
• Advocates for clients to access housing, services, and supports while ensuring funding compliance.
